 Description
   This error occurs when executing a menu which has not been previously
   initialized. The two common reasons are to call the menu to command
   without previously using the prompt command, or to call the menu to
   command repeatedly without specifying the save or read subcommands
   for the prompt command (in that case the menu is cleared after the
   menu to command returns).

 Solution
   Use the prompt command to initialize a menu prior to using the menu to
   command. Use the save or read subcommands if a menu is to be used more
   than once.
